Include a completed requisition form for each sample in the shipment to avoid delays in sample processing.

Online Order
Pay for your tests
Select your tests and place an online order. You will receive an invoice for credit card payment.

Shipping Specimens
Ship samples
Ensure specimens are collected according to our specimen collection guide.
Follow the shipping specimens guide for timely receipt and processing of samples.
